continual volunteer form

This form is to be completed before you participate in any volunteer opportunities with the Beaverton Downtown Association (BDA). It will be kept on file perpetually but for no less than three years, although in some cases you may be required to complete and submit a new version.

WAIVER & PERSONAL RELEASE

Please read carefully before submitting below.
In consideration of BDA providing the participant named above with the opportunity to volunteer at any opportunities with BDA following the signing and submission of this form, I hereby agree to the following terms and conditions:
To participate as a BDA volunteer, I understand that the participant must:

• Follow all safety instructions and procedures presented in connection with each BDA volunteer opportunity.
• Use all necessary precautions to protect against property loss or damage, bodily injury, and death.
• Immediately stop participating if any activities become too strenuous, difficult, or hazardous.

RELEASE OF LIABILITY

• I understand that as a volunteer, I am not covered under a worker’s compensation program of any sort, and that I will be personally responsible in the event I am injured while performing my volunteer duties.
• I understand that I am authorized to drive my personal vehicle as part of my volunteer duties and that I will be personally responsible for covering bodily injury to other or property damage resulting from an accident. I will only drive with a valid driver license and auto insurance while volunteering.
• I understand that criminal background checks are conducted on all adult volunteers responsible for handling money, working with the public, or accessing confidential information. All applications and results will remain confidential.
• I grant to Beaverton Downtown Association the right to use my name and image for the promotional, educational, and business purposes in all forms and media, and waive the right to inspect or approve versions of my image used for publication or the written copy that may be used in connection with the images.
• I certify that all the information provided on this application is correct.

I represent and warrant that:

• I am the participant listed above, or I am the legal guardian of participant listed above.
• The participant is physically able to participate in volunteer opportunities for which participant is registered.
• The Participant’s involvement in BDA volunteer opportunities is completely voluntary, and the participant has received nor expect to receive any compensation for participating in it.
• I agree and comply with this waiver and will not take any actions that would assist or cause the participant to violate, invalidate, renounce, negate, revoke, or disclaim any part of this waiver.
• All the information provided on this form is correct.
